We recommend that you arrive 15 minutes prior to you appointment time in order to register an go through and necessary screening for your exam. Please make sure to bring the following with you:

  • The written order from your referring physician
  • The name and address of any additional physician that you would like copied on your exam results
  • Your insurance card (note: if your exam requires pre-authorization through your insurance, please ensure that this is obtained prior to your exam)
  • Your copayment if applicable (we accept Mastercard, Visa, cash, check or money order )
  • If your exam is going through Worker’s Compensation, please have the name and address of the employer and Compensation carrier as well as your date of injury
  • If your exam is going through No Fault, please have the name and address of the No Fault insurance carrier as well as your accident date
  • You will need to bring a driver if you are receiving sedation
